Ozzy Osbourne’s cause of death has been revealed

Two weeks after Ozzy Osbourne‘s death, the Black Sabbath frontman’s cause of death has been revealed.

According to a death certificate obtained by The New York Times, Osbourne died of a heart attack “out of hospital” and suffered from coronary artery disease and Parkinson’s disease. The certificate, filed by his daughter Aimee Osbourne in London, lists his occupation as “Songwriter, Performer and Rock Legend.”

The certificate further states that Osbourne died of “(a) Out of hospital cardiac arrest (b) Acute myocardial infarction (c) Coronary artery disease and Parkinson’s disease with autonomic dysfunction (Joint Causes),” according the Times.

Osbourne died July 22. An air ambulance flew to his family’s home near the village of Chalfont St. Giles in Buckinghamshire, England. His family announced his death that day without cause, though he had publicly revealed the complications to his health due to his Parkinson’s diagnosis. He had announced in February that he could no longer walk.

“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” Osbourne’s family said in a statement confirming his death. “He was with his family and surrounded by love. We ask everyone to respect our family privacy at this time.”

Earlier in July, Osbourne joined the original lineup of Black Sabbath for a star-studded farewell show. He performed from a throne.
